TRANSPORTATION INFORMATION

Below is information concerning transportation services provided by Lassen Senior Services to the Big Valley senior citizens.

Our requested donation for transportation to the Big ValleyNutrition Site for congregate meals (lunches on Wednesday and Friday) is $.50. Please note - should you not be able to make a donation, you willstill be transported to our nutrition site.<>
Our requested donation for out-of-town trips for medicalappointments, etc. is $15.00 for seniors and the disabled, and $20.00 for
non-seniors and those who are not disabled. Once again, if you are a senioror disabled and unable to make a donation you will still be transported.
No out-of-town passenger is ever required to contribute to thedriver's lunch. This is completely voluntary.

INFORMATION CONCERNING SERVICES IN THE BIG VALLEY AREA

Please be advised that Lassen Senior Services will be providing the
following services to the Big Valley Area:

Homebound Meals (Meals On Wheels)
Out-of-town Medical Transportation

It is necessary for us to temporarily suspend congregate meals at the Bieber Nutrition Site effective 9/15/2005 until a Site Manager can be hired. We anticipate that congregate meals will resume within two to three weeks after that date.

We had planned to send personnel from Susanville to act as Site Manager. However, because the environment at the Bieber Nutrition Site is so hostile to Lassen Senior Services administration and staff, we will not send anyone to this site alone. Staff at the Susanville site has been reduced to three people. We cannot send 2/3 of our staff to Bieber two days per week without jeopardizing the agency's ability to properly administer the agency's other three sites, as well as accounts payable, accounts receivable, payroll, grant monitoring, transportation reporting, and all of the other administrative duties needed to keep things running smoothly.

We will advise you by mail and newspaper advertisements when congregate meals will again be served.

INFORMATION CONCERNING LASSEN SENIOR SERVICES, INC.

Lassen Senior Services, Inc. is an independent, non-profit corporation formed for the purpose of providing services to the elderly citizens of Lassen County. The agency is funded solely by grants and donations.

All of the services provided to the Big Valley area are paid for by Lassen Senior Services. This means that Lassen Senior Services

Pays the salaries and associated payroll taxes and social security contributions for all site personnel.
Purchases all food and food service supplies.
Pays for the liability insurance necessary to allow the site tooperate.
Pays for vehicle insurance for the Big Valley van.
Pays for fuel and maintenance of the Big Valley van.
Buys all office and other supplies.
Pays for the telephone in the office.

In return, Lassen Senior Services requires that the site adhere to all policies, procedures and directives developed by their Board of Directors as outlined in grant parameters and contracts with the County of Lassen.

As a point of information, the site menu and food preparation has always been at the sole discretion of the Site Manager. Lassen Senior Services neither directs nor controls them. Our only requirement is that they meet FDA standards.
